X на учете = X is registered

X is on the books X is numbered (in some records etc)

X на учете в милиции - X has a (police) record

X на учёте в психиатрической больнице (туберкулёзном диспансере и т. п. ) = X is under psychiatric observation

X is registered as a mental (ТВ etc) patient (case).

«Хочешь подробности о ней узнать, зайди в детскую комнату шестого отделения милиции. Она там на учете стояла» (Черненок 1). "If you want to know more about her, drop by the juveniles' room of the Sixth Precinct. She has a record there" (1a).
